Cause: Faulty selectable one-way clutch
Correction: Replace the clutch

Additional Notes: These selectable one-way clutches are solenoid controlled to be applied and released. They have struts that apply and release the clutch, and these struts can wear into the plate and effect the apply of the clutch. Check the attached pictures, they show the wear points and how even a little wear can affect the clutch operation.
